Cervical myelopathy, radiculopathy

- Cervical spondylosis = degenerative changes
- osteophyte, foraminal stenosis: may cause radiculopathy
- changes at margin of vertebral bodies and post longitudinal ligament : may cause cervical stenosis and myelopathy
- Herniated disc
- lateral herniation: may cause radiculopathy
- large midline disc: may cause myelopathy
- Nondegenerative causes: infection, nerve root infarct, infiltrate by tumor, granulomatous tissue, demyelination
Cervical radiculopathy
- Study of 561 cases
- Mean age at Dx: 48 years old
- Incidence highest for 50-54 years old
- C7 most common 70 %
- C6 about 20%
Danger signs
- Lhermitte phenomenon: neck flexion -> shock like paresthesia
- Fever, chills, unexplained weight loss, immunosuppression, cancer, iv drug use: may suggest tumor, infection
