Cervical myelopathy, radiculopathy

  • Cervical spondylosis = degenerative changes
    • osteophyte, foraminal stenosis: may cause radiculopathy
    • changes at margin of vertebral bodies and post longitudinal ligament : may cause cervical stenosis and myelopathy
  • Herniated disc
    • lateral herniation: may cause radiculopathy
    • large midline disc: may cause myelopathy
  • Nondegenerative causes: infection, nerve root infarct, infiltrate by tumor, granulomatous tissue, demyelination

  • Study of 561 cases
  • Mean age at Dx: 48 years old
  • Incidence highest for 50-54 years old
  • C7 most common 70 %
  • C6 about 20%

Danger signs

  • Lhermitte phenomenon: neck flexion -> shock like paresthesia
  • Fever, chills, unexplained weight loss, immunosuppression, cancer, iv drug use: may suggest tumor, infection
